All the providers that came to my home were polite, professional, and pleasant. Danielle, Tracy, Tammy, Daniel, Jesse, and Quincy were ones who provided care for me. Although I am happy to have reached my goals and I’m progressing. After breaking my femur and being in rehab it was suggested that I have in home physical therapy. Triniti Home Health sent out both a physical and occupational therapist to get me back on track. Peter was so great at teaching me how to get around at home and manage my daily activities. He was a joy to work with and gave me strength and confidence even on the hard days. So thankful for him. I highly recommend.

What is home health care?
Home Health encompasses a wide range of health and social services for individuals recovering from an acute illness or managing chronic illnesses. These services are delivered at home to recovering, disabled, acute, or chronically ill individuals whose needs may be medical, nursing, or therapeutic treatments.
Our staff travel to the client’s homes which include single-family houses, apartments, assisted living facilities, group homes, residential housing facilities, or congregate living environments.
When is home health beneficial?
Home health is beneficial to individuals recovering from an illness, injury, or surgery. It’s also often an alternative to long-term institutional care. Oftentimes, families may require higher levels of ongoing support for their loved ones and that’s when we step in. Home health services allow clients to maintain independence and privacy as they receive regular support from their team at home.
How does one qualify for skilled home health care services?
To receive our home health services, a physician must verify that the individual is homebound, and write orders for home health including skilled services such as nursing and physical therapy. Individuals who are interested in home health services should contact our home health team to discuss their eligibility.
Who pays for home health services?
Medicare, Medicaid, and most private insurances pay for home health services. Skilled services are covered 100% and are delivered on an intermittent basis.
How often does a home health professional come to the home?
Each home health client receives a personal care plan tailored to their needs. It’s common for many clients to receive multiple visits during the week depending on which services they are receiving. Each visit usually lasts 35-45 minutes.

What is hospice care? Why should I consider hospice?
Hospice refers to the philosophy that focuses on providing palliative care to individuals at the end of life. Hospice care focuses on comfort and quality of life until death, instead of curative treatments that may postpone death or diminish the quality of life.
What is the goal of hospice?
The goal of hospice is to focus on symptom management and quality of life, so individuals and their families enjoy the remaining time they have together. Hospice also provides training to the caregiver in the home on what to expect with changes at end of life.
What does hospice offer?
Our hospice team of interdisciplinary members offers excellent pain and symptom management, respite, as well as socio-emotional and spiritual support for individuals and their families. Support is provided 24/7, there is always someone available to answer questions and to make a visit. In addition, Triniti Home Health & Hospice provides bereavement services for families who have lost a loved one. Over a 13-month period, the bereaved will receive resources and support from our coordinator and team.
How does a person qualify for hospice?
To qualify for hospice, a doctor must indicate that you have a life-limiting illness that limits life expectancy to less than 6 months if the disease were to follow its normal course. Anyone with a life-limiting illness such as heart disease, lung disease, end-stage dementia, cancer, or other chronic diseases could qualify for hospice care. Contact us for more information!
What services are offered by hospice?
Hospice services are designed to provide emotional and spiritual support to individuals and their families. In addition to the core team of caregivers consisting of registered nurses, a physician, and a social worker, someone may choose to receive additional services from a home health aide, chaplain, volunteer, music therapist, and massage therapist. Triniti hospice also trained the care team to provide M-technique, a special comfort measure, as well as aroma care.
What qualifications and training do hospice staff have?
In addition to being compassionate and having a calling to serve clients and families during end-of-life, all hospice staff are licensed/certified and have received specialized training to administer end-of-life care. In addition, Triniti Home Health & Hospice nurses and social workers have been trained specifically for veterans at end of life (level II We Honor Veterans provider). Also, our medical director has special hospice medical certification and is available to us 24/7.
Who pays for hospice care?
Hospice is a Medicare benefit. Anyone who has Medicare Part A that meets eligibility can receive hospice services paid for by Medicare.
Aside from Medicare, medical assistance and most private insurance programs also pay for hospice services. This typically means that cost of services, supplies, medications, and equipment related to the individual’s terminal diagnosis are covered. Hospice provides different levels of care depending on the stage of illness and does not typically provide 24-hour care. If your loved one requires round-the-clock care, family members may need to pay out-of-pocket for additional services.
Does a hospice organization need to be certified?
Yes. Hospice is licensed by the Minnesota Department of Health and certified by CMS the Center for Medicare Services. Triniti Home Health became Medicare certified in 1991, and Triniti Hospice became Medicare certified in 2005.
